High School Student Permission Form

All high school students who are not accompanied by a parent or guardian, or who do not possess a Rice University ID must email a scanned copy of this completed and signed form before visiting Fondren Library.

• Print this PDF and fill out the form.
• Have it signed by the high school librarian.
• Please scan and send the completed form via email to flsd@rice.edu a minimum of 3 days in advance of the visit for verification.
